How to Obtain a SIDA Badge (Initial and Renewal Badges and must have already established company with the Security Badging Office)

A. Initial Badges

1. For initial (new) badge holders, the security badging process takes place in 3 steps: applicant record building/fingerprinting, training and badge issuance. During the initial step of the badging process, the applicant submits the following to the badging office staff member:

3. Complete the application forms and obtain the necessary signatures. Please remember these basic instructions when filling out the application:

  • Complete the form using BLACK ink only
  • Type or print information legibly
  • ALL correction tapes/fluids are prohibited – if you make a mistake, please print and complete a new form
  • All three sections must include original signatures by the necessary parties – stamped signatures are not accepted
  • Applications must be presented to the Security Badging Office within 30 days of a sponsor’s signature
  • Do not fold or staple the application

In addition, use the following information as a guide for completing each of the applications:

Use the following information as a guide for completing each section on the application:

  • Top Section
    • Must be completed by the applicant with an original signature and date
  • Middle Section
    • Must be completed by an Authorized Signer designated for that company
    • New companies must register with the Security Badging Office. See NEW COMPANY SETUP.
  • Aircraft Operator Certification Section (SIDA applicants)
    • If employed by an airline, this information is obtained through the employer.
    • An authorized signature is needed, granting/authorizing access
  • Bottom Section
    • To be completed by the Aviation Department

STA APPLICATION (SECURITY THREAT ASSESSMENT)
Use the following information as a guide for completing each section on the application:

  • Top Section
    • Must be completed with the applicant’s personal information
  • Middle Section
    • If the applicant is Not a citizen of the United States, he/she will complete the first boxed section
    • All applicants must complete the second boxed section using information provided on Tables 1 through 3 detailing the list of acceptable documents
    • All documents listed must be provided as originals and unexpired to the Security Badging Office for review in order to proceed with the badging process
  • Bottom Section
    • Applicant must review the information and sign with an original signature and date

CRIMINAL HISTORY RECORDS CHECK (CHRC)
First time applicants are required to clear an electronic, fingerprint-based criminal history records check before a badge can be issued. Applicants are required to read, sign and answer each question individually on the CHRC Application before receiving fingerprint services. This application lists the offenses (based on conviction) that disqualify applicants from receiving a badge.

4. Turn in your completed application forms with proper signatures to the Security Badging Office. At the time of submission you will be fingerprinted (if required).

5. Documentation and Identification required at time of application submission. All documentation and identification MUST be original and unexpired.

a. Documents / ID required to satisfy the Criminal History Records Check (CHRC)

Two (2) forms of identification are required:

  • One (1) MUST be a valid, non-expired, government-issued photo ID, such as a driver’s license, military ID, or passport. If applying for airport driving privileges, you must present a state issued unexpired driver's license issued within the United States.
  • A second identity form includes any of the above, social security card or birth certificate.
  • Non U.S. citizen applicants MUST present their employment authorization documentation. An Alien Registration Card must be presented for verification and a Non-Immigrant Visa (U.S. issued, found inside passport) must be presented if applicable. Refer to requirements on the STA Application form.

b. Documents / ID required to satisfy the Security Threat Assessment (STA)

Refer to the STA Application form for the documents and / or identification requirements.

6. Background Checks

The company will be notified once the Security Threat Assessment and Criminal History Records Check clearance are received. Applicants have 30 days from notification date to return to the Security Badging Office to attend the applicable training course and to claim the badge. Airline-sponsored applicants will be notified of this clearance through the carrier. Failure to return within this time frame will cause the application to expire, resulting in employees to reapply and start the process over with additional fees. Reminder: applicants cannot be issued a badge until the background checks have been cleared.

7. Security Training

This is mandatory training for the applicant, except for those whose work related duties require a public badge, and must be successfully completed before a badge can be issued. PHX uses an INTERACTIVE COMPUTER-BASED TRAINING PROGRAM and the training schedule is below:

  • Monday through Friday: 8:30 a.m. – 4:30 p.m.
  • Cut-off time to begin training is 2:00 p.m.

B. Renewal Badges

Badge-holders are required to renew their badges within 30 days prior to badge expiration. Failure to renew within this time period will result in badgeholder retaking the security training classes before the badge can be issued. Failure to renew after 30 days from badge expiration date will require the employee to complete the application process from the beginning as a first-time applicant, including the fingerprinting process and security training.

3. Turn in your completed application forms along with original and unexpired documentation and/or identification (to satisfy CHRC and STA requirements) to the Security Badging Office. Once confirmed that all background checks, documentation, identification, training, service fees, etc. are in order, the renewal badge will then be issued to the badge applicant at the time of application submission.

Failure to comply with any of the requirements mentioned above for the badge renewal process will prevent the issuance of the renewal badge.
